Dual Diagnosis Outreach Project

The purpose of this program is to provide outreach and short-term intensive case management to persons who are homeless with mental illness and alcohol/chemical dependency. The goal is to assist them in achieving community stability and reaching their fullest potential. Services include:

  • Camp/street/agency outreach
  • Assistance finding safe, short-term emergency housing
  • Linkage to long-term treatment and case management
  • Substance abuse counseling
  • Mental health counseling referrals
  • Psychiatrist and nursing care
  • Medication funding and monitoring
  • Psycho-social rehabilitation and life skills training
  • Access to a 24-hour crisis line
  • Assistance with Section 8 applications and other long-term housing resources
